American Medical and Life Insurance is committed to providing a positive experience for all of our members. To that end, we are in the process of creating a consumer self-service environment for our plans. Once complete, all data surrounding claims and benefits will be available at www.usamli.com. Check back for updates as they become available.
In the meantime, claims and benefit inquiries can be made by contacting our Customer Service Center at 888-264-1512. When calling, please have your membership information ready, including your Group ID and member number found on your ID card.

Health Advocate – Patient Advocacy for Limited Medical Plan Members
AMLI has partnered with Health Advocate to enhance the benefits provided to you under our Limited Medical plans. Health Advocate is a service that doesn't cost you anything. AMLI makes it available to help you and your family find answers to medical and health care issues, help resolve medical billing issues, file appeals, and address other insurance-related concerns. Health Advocate can help you to:
