Notice to UTAH members of AIM Limited Medical Benefit Insurance
If you are a resident of UTAH who purchased limited medical insurance through membership in the Association of Independent Managers (AIM), underwritten by American Medical and Life Insurance Company (AMLI) and your coverage was terminated on 12/31/09, you may have been entitled to purchase an individual conversion policy directly with AMLI.
A letter explaining the details of your right to covert your policy has been sent to uninsured UTAH members. If you have any questions about the coverage being offered, please contact our Customer Service Department at: 1-877-539-3940

False Certificates of Creditable Coverage
Certificates of creditable coverage purporting to reflect insurance by American Medical and Life Insurance Company for members of the Association of Independent Managers after December 31, 2009 have been issued without the Company’s authorization. The Association of Independent Managers terminated the policy issued by American Medical and Life Insurance Company as of December 31, 2009. Any certificates of creditable coverage which represent coverage following that date are inaccurate.
If you believe you were insured by American Medical and Life Insurance Company and that you are entitled to a certificate of creditable coverage, we recommend you contact us at 888-264-1512 to request a certificate of creditable coverage.
